An adequate data-flow in Primary Health Care (PHC) and between PHC and Secondary Care is essential in the realisation of a cost-effective health care system in European. An equitable access to PHC for all citizens, including socially vulnerable groups, is a challenge for the coming years. The implementation of tlematics can contribute to the realisation of these goals. The objectives of the concerted action were to consider both the conceptual and technical problems related to its implementation (classification of data, adequate hard- and software), the contextual basis for the development of telematics in PHC with regard to ethical, social and organisational impact and consumer perspective, and the identification of areas where further development is needed to meet the problems of general practitioners. The main achievements were a precise description of the "state-of-the-art", a thorough analysis of the enabling and limiting factors for telematics in PHC and specific recommendations for further European action.
