Overview

The MEDIREC was a consensus building Concerted Action of representatives of all Member states to achieve on a European level a common understanding of the electronic patient record and to elaborate a number of strategic guidelines and recommendations that will contribute to the availability in Europe of electronic patient records that are comparable, exchangeable and commonly understandable and interpretable over national borders and among different linguistic communities.

Purpose and objectives

Several approaches to electronic healthcare record systems (EHCR) have been attempted, with different degree of success, over the last decades. However, no complete EHCR exists today, if a complete EHCR is defined as a system that can contain all existing patient information according to the clinicians wishes, represented in a way that makes the information reusable. Before the MEDIREC project, very few developers or even users were aware of this. Also, the market of EHCR was very fragmented.

The main objective of this Concerted Action has consisted of having on a European level a common understanding of the medical record in order to ensure that medical records coming from different origins are reliable, communicable, recognisable and comparable. The work method has been developed through six work-package, independent among them. Now you can find described briefly the main function of every work-package.

Preparation of "the European Electronic Medical Record Index" This work-package has tried to give a clear description of Guidelines for the creation of a work of reference.
Workshop MIE 94 This work-package was used to present to the CA work group the first work-package for discussion.
AIM Euroforum Workshop The main goal of this WP consisted of amendment WP one starting from all suggestions made at MIE94.
"Permanent structure directives" The main objectives of this work-package were dedicated to reach a permanent structure.
Document "Strategic guidelines for further actions to be taken" This document proposed "the strategic guideline" in the field of EMR to orient the future actions to be taken.
Closing Workshop (MIE AMICE 95, Amsterdam) The main goal of this work-package was organised in order to present the results of the CA.

Results

The MEDIREC project succeeded in identifying the key issues that needed to be addressed to open the market for EHCR:

standards for electronic healthcare record (systems) should be made largely known and implemented
the standardisation effort should enable the interoperability at three levels: within the complete information system of the health care organisation, between two different instances of a same EHCR system, located within different health care organisations, and even within different countries, between two different EHCR systems. It should also allow to customise and adapt the solutions to the local uses, rules, languages and customs.
harmonised healthcare rules and regulations: ideally, there would be few differences in national rules and regulations. The differences that are to be maintained, should be well defined and specified, so that the effort needed to make different national versions of the systems is reduced to a minimum.
support of European suppliers: As in other sectors, the new market requires that all actors are equipped to participate successfully, or at least that they do not start with significant handicaps. All should be able to operate according to clear rules, within a single, fair and competitive framework. European suppliers, often SME's, are, unlike big non-European companies not used to compete in a wide and open market. They may however, possess both valuable knowledge and interesting products made for smaller markets, that it would be valuable to make available on a broader scale. To have them acting in an open European market, it is necessary to provide them with a support, both in order to obtain the necessary level of investment and to guide their entry into this market..
consensus on what should be understood by a European Healthcare Record: it is necessary to establish a European consensus on what a EHCR system is, what it can do, how it shall do it, and what it may and should contain.
In addition, the project was able to convince a large numbers of actors in the field to move forward in one direction. Complementary evolutionary pathways could be followed, but the long-term objectives of new developments should be similar.

Finally, in order to ensure that the exploitation plan on the field of EHCR has a high degree success, to maintain the following general lines have been recommended for getting the operational goals of MEDIREC project, that are:

to establish a permanent structure
to define actions
to identify the users groups generations
and finally to define professional standards
