Overview

Laparoscopic surgery (surgery by minimal access, internal view by video camera and miniaturised instruments) is a new surgical technique, which offers major benefits for the patient (reduction of postoperative pain and in hospital stay). Video-imaging is intrinsic to the technique. Teaching and training are thus facilitated, as “ the student sees what the surgeon is seeing during the procedure ”.

The aim of the TESUS project is to federate European University Hospitals (Belgium, Germany, France, and Switzerland) in a network for on-line transmissions of surgical procedure imaging and counsel in day-to-day practice. European staffs organised every week on specific surgical topics and regular transmission, for teaching purposes, of live surgical procedures will allow for an approach to standardisation of practice across the continent.

The specific creation of work methods and tools adapted for handling surgical video Information, will be an important step in a global European approach to health policies in the specialised field of tele-surgery.

Purpose and objectives

Developments in the project will be just as apt for any other application area wich imposes the use of high-quality video information, be it in the medical sector or elsewhere, due to the high level demand of surgery in image quality. Examples are university exchange programs, optimised standard videoconferencing and other high technology industrial fields (car manufacturing, aerospace industry,..). The opportunity for technology transfer is great. By offering broader dissemination of knowledge and know-how in laparoscopic surgery, the TESUS project will optimise patient care and be a participating factor in the reduction of health care costs in the EU.

Video-data input into a surgical data base for teaching and training is also a goal of the project, as it will permit the edition of “ golden ” standards in the speciality, reached by consensus.

TESUS represents a real step for surgery in general and in minimal access surgery in particular. It was the first time that an expert could practice directly on a case at several hundred kilometres from his work-place. But TESUS is also there to allow for reflection on the ethical and legal problems of using Tele-transmissions for the treatment of cases.

Results

TESUS permitted the creation of an active network for surgical staff. Beyond the technical specification which was necessary to create the links, TESUS was a real challenge for the surgeons to call into question their practices. After 25 months work, the TESUS network now attract surgeons throughout Europe and all over the world. Word-wide connections are made weekly, and the TESUS network is known throughout the world.

A majority of surgeons has understood the ergency of call into question their practices, and they want to obtain the best information from the best expert in the case dealt with, in the shortest possible time span.

From now on, minimal access surgical staff can suggest investments to their hospital management.Via the TESUS network system these investments will be econmically worthwhile. firstly by the information on surgeon avelability to resolve problems in delicate cases (Expert or another surgeon on the network).

The TESUS network is the first step to surgical practices of the third millenium, such as Telesurgery, laparoscopic surgery, interactive teaching, simulation procedure, electronic surgery publication.
