Overview

The DIABCARD project aims at implementing and testing a chip card based medical information system (CCMIS) for chronic diseases in ambulatory and hospital care. The smart card serves as a portable computer-based medical record. The medical record is central to the management of patients. Due to the variety of the illness a person with a chronic disease mostly has many records, held at different locations. This often poses a problem. Having a computer-based patient record on a smart card makes the patient's up-to-date record accessible when and wherever it is needed.

The project is in its third project phase now. In phase I the system was specified and technical demonstrators and evaluation tools were developed. On the basis of the commercially available systems Millenium and Diabcare® DIABCARD systems were prototyped and field tested in Athens, Barcelona and Umbria in the 2nd project phase. It could be demonstrated that for a diabetes speciality record the capacity and functionality of the chip card is sufficient. The main emphasis in DIABCARD 3 is on implementations in the project partner countries. The implementations are to be seen as a first step to a routine use of the system and to large scale implementations. Another main task of DIABCARD 3 creating the preliminaries for a smart card that is inter-operable to the Cardlink smart card.

Purpose and objectives

The long term goal of the project is to improve the quality of medical care in chronic diseases. Diabetes mellitus is being used as a paradigm including nearly all aspects of chronic health care. On top of that diabetes mellitus concerns about 5-8 % of the European population and it causes many secondary diseases as for example acidosis, nephropathy up to acute renal failure, vascular diseases or blindness.

The chip card acts as a communication tool. The communication between the different health care professionals and institutions is being improved in routine and also in emergency medical care. The patient is involved as an active partner. DIABCARD is enhancing standardisation of medical documentation thus supporting effective management of diabetes mellitus. The application of the DIABCARD system will improve the quality of diabetes care. Secondary diseases can be avoided or at least the onset delayed. This means reducing healthcare costs of insurance companies and governments.

Great care has been taken to develop a useful and usable system. Therefore users have been involved right from the start. At the same time existing and emerging standards have been integrated. It is hoped to eventually create a health smart card network all over Europe.

Results

The DIABCARD CCMIS architecture is flexible and adaptable to technological advancements of the chip card. It incorporates the interoperability specifications of the G7 Healthcard Interoperability Group [Ref. web site http://clinical-info.co.uk/euhci.htm], the PC/SC Interoperability Working Group and of the OpenCards Framework. It allows interoperability on card-reader, on chip card level and on data level. A high level application interface (API) was defined which relieves application programmers from handling smart card specific problems like communication and security protocols, card versions, local or remote access and performance optimisation.

DIABCARD can be used as a stand-alone system or it can be integrated into existing information systems and networking environments. An existing network infrastructure is not necessary. The patient's self monitoring device can easily be integrated. The smart card acts as an additional data storage and management tool (emergency data, patient's medical history data) on the one hand, and as a communication tool on the other hand. It connects, for example, the GP doctor's office system to other parts of the health care network.

DIABCARD has developed a data set. Version 2.1 is available. This data set comprises administrative and medical (emergency and diabetes speciality) data. The administrative and emergency data is inter-operable to the Cardlink and G7 Healthcard Interoperability data. The data items have been grouped logically. This supports data integrity as well as storage optimisation. The data set is represented in ASN.1 BER [ISO 8825 Specification of Basic Encoding Rules - Abstract Syntax Notation One]. For the data architecture the layered approach was chosen: a basic layer with an administrative set, a second layer of an basic emergency data set and vertical areas of tagged clinical information.

The final system will have a 'Patient Card' and a 'Professional-Card'. The main purpose of the latter is to identify the health professional to the information system, including the medical record, i.e. to the Patient Card. The access rights of the health professional are specified in an access matrix. DIABCARD data will not leave or enter the patient’s card without the verification of the health professional's access rights. Emergency data can be read without any restrictions, but it can only be entered according to the rights laid down in the access matrix. On the subject of data security (backup) or ‘Trusted Third Parties’ DIABCARD is co-operating closely with other on-going EU projects, for example TrustHealth.

The DIABCARD system is being implemented in Austria (Vienna), France (Corbeil), Germany (Munich, Kassel), Greece (Athens, Thessaloniki), Italy (Umbria) and Spain (Mataro). The chip card serves as the communication tool. Different communication models are envisaged. Communication between a Diabetes centre and GPs, between hospital (ambulatory care), GP and specialist, and between Diabetes Centre and home care will be performed. The organisational, legal and cultural differences of the various healthcare systems have been taken into account. All pilot sites have been primed and the implementations are scheduled for 1998.

We are sure that DIABCARD will have a major impact on the use of information systems and on diabetes care.
