Overview

The major objective of the project is to "develop interworking between professionals involved in medical assistance, in order to improve the continuity and quality of health care across borders and in remote areas".
This will be achieved by designing, developing and validating the "ET-ASSIST Information System" which is an open, secure, multimedia medical platform, based on INTERNET technologies and standardized Electronic Forms Interchange between healthcare professionals and assistance providers.

Purpose and objectives

The main objective of the system is to propose a communication service between two well defined professional communities : Assistance Providers and Healthcare Professionals. In the project, these two communities are defined as follows :

Assistance providers

Assistance providers have the aim of solving immediatly any (medical) problem which may arise for a traveler or an expatriate professional who has (as a private physical or moral person) subscribed an assistance contract. In the project, such a traveler or professional is called an "assistee".
The know-how of the assistance provider consists in its skill to organise the provision of care according to the mandate contractually agreed with the assistee, including, when necessary, repatriation.
An assistance provider does not provide healthcare directly. It subcontracts to external service providers (ambulances, airplane companies, taxis, hospitals, physicians etc...) as necessary and coordinates with the healthcare providers responsible for the traveler facing a medical problem.
Assistance companies participating to the project collectively manage assistance contracts representing more than 30 millions of individuals, mainly from UK, France and Germany.

Health Care Professionals

Health Care Professionals (HCP) involved in the project are defined according to the CEN/Technical Committee 251 "medical informatics" standard definition :
"a person who is entrusted with the direct or indirect provision of defined healthcare services to a subject or a population of subjects.
Example : primary care physician, dentist, nurse, social worker, veterinary surgeon."

Participating HCP are working in the regional health networks of French Guyana, Auvergne of "Vallée de la Tarentaise" in France, or the Island of Crete in Greece, the Maritime Medical Advice Centres of Spain, Greece and France, and the assistance companies themselves.

The system shall also propose "medical assistance case management" services. The life-cycle of a medical assistance case is in average 48 hours, but can last up to 1 or 2 months.

Results

The first factor for the successfull deployment of the targeted system is its dissemination among the Healthcare Professionals community. The use of INTERNET / WEB technologies has proven to be the only solution to preserve a dissemination potential equal to the number of healthcare professionals in Europe.

Assistance activity is usually marginal for Healthcare professionals. Most of the investment costs necessary for the deployment of the final system shall be supported by the Assistance Providers.

The system will effectively improve the continuity and quality of care if it can manage and transmit medical multimedia pieces of informations produced by the heterogeneous information systems of the Healthcare Professionals. The selection and when necessary the development of low cost standards based software components and the specification of standard interfaces and data model is expected to solve interoperability issues. The project will demonstrate this through the interconnection of the IS of the pilot sites which all have different systems. Guidelines for interconnection will then be validated. The first version of the final ET-ASSIST Information System is scheduled to be operational in May 1998.
