Overview

The diagnoses and prognoses of most severe diseases, require microscope examinations of a biopsy of the affected organ in order to evaluate the deviation of tissues, cells and chromosomes from normal. Observations are often complemented by analysis of biological markers for more specific diagnosis and prognosis. The annual activity in EU is 200 million microscopic preparations examined by highly dedicated specialists in 3 000 laboratories for a cost of 2 Billion Ecus. As shown by numerous initiatives making use of telematics in pathology examinations, « telepathology » is already an highly promising and realistic application of telemedicine which is fostered by the project. in two directions: (1) the integration of multimedia telecommunication between pathologists to improve quality assurance, diagnostic accuracy and cost efficiency of screening, diagnosis and prognosis in routine practice. In this respect, the project provides the inter-operable tools for remote consultation of experts, consensus diagnostic through microscope teleconferencing, and remote quantitation of biological markers; (2) the integration of multimedia European databases for evaluating the prognostic value of the large amount of biological information now available on tissular lesions as it comes from the rapidly increasing number of immunological markers and genetic probes in use. The project will establish compatibility and inter-operability between the major European manufacturers of microscope and designers of imaging workstations and will issue the medical protocols and guidelines to support the set-up of telemedicine European services offering added-value in pathology.

Purpose and objectives

The project aims at pathologists in university hospitals, district hospitals and private laboratories. The potential is 13 000 medical sites in EU. For the first time, these medical actors will access to a real-time second opinion on difficult cases, to banks of reference images, to electronic records of reference cases and to up-date evaluation of prognostic biological markers thus enabling quality assurance and quality control procedures for the diagnosis of severe diseases. Best diagnostic expertise will then be available independently from geographic location within Europe.

The Project core technology is the microscope imaging workstations networked through LAN, WAN (EuroISDN) and Internet and will integrates ATM potentialities as they become realistic. The project actually federates the European industries distributing microscope workstations in order to reach full inter-operability. This will thus strengthen their competitiveness on the expanding world-wide market of telemedicine.

The 3 key objectives of the project are:

  1. The integration of multimedia communication software tools between pathologists to improve: quality assurance, reproducibility, accuracy and cost efficiency of pathology routine diagnoses and prognoses.
  2. The implementation of multimedia European databases for storage and retrieval of: Pathology Case Reports jurisprudence, Reference Macro- and Microscopical Images and Markers provided by Cytometry, Cytogenetics and Biochemistry.
  3. The design of inter-operability standards in the direction of manufacturers and the design of inter-communicability guidelines in the direction of the end-users in the project domain.

Results

The main concrete achievements of the project are:
  1. Three pieces of software have been developed:
  2. These software packages are available as industrial product which distribution is in progress (50 systems installed already).
  3. The inter-operable interface to motorized automatic microscope have been made available in the meantime by Carl ZEISS and LEICA. Their high cost is presently limiting the dissemination although 2 partners in EUROPATH consortium have been already equipped.
  4. The design and implementation of transcoding tables (ADICAP/SNOMED/ICD) for multilingual electronic data exchange (English, French, Spanish) in most domains of medical pathology.
  5. The set-up of a high capacity EUROPATH Web Server using a dedicated UNIX based system (UltraSpark 1, SUN 170, 2.1Gb, 167 MHz driven by UltraSpark 1, SUN 140, 1.05 Gb with Multipack disk 4.2 Gb ext. up to 25 Gb).
  6. The implementation of the LOTUS NODE Web server software support.
  7. The design and implementation of a common database structure for case reports, reference images and biological markers within the ORACLE Database shell.
The project has contributed to foster the set-up of an « Oncology Network » in the framework of the G7 Global Healthcare program.
