Overview

MERMAID attempts to establish a transnational and multilingual health emergency system that improves the effectiveness and ease of delivery of telemedical intervention and also improves the present state of connectivity of the emergency points of care with the providers of telemedical services. Thus MERMAID provides an integrated 24-hour multilingual world-wide medical emergency service and uses telematics for making available high grade medical expertise to sea-borne vessels.

Purpose and objectives

Council Directives 92/29 and 93/103 establish the “use of long distance medical consultation” for the protection of the safety and health of maritime workers and distant or isolated populations of the EU. In response to these needs, MERMAID has set up an integrated 24-hour multilingual, telematic, around-the-world, medical emergency service that transfers medical expertise via satellite and ground based ISDN networks when and where necessary. The MERMAID service is based on:

A locally resident multimedia medical guide for ships to serve both for training and as a reference during emergencies
A system of INMARSAT (ship to shore) links (for transfering images, sounds, text / patient anamnesis and signals), which is feeding into an ISDN network that links together the MERMAID health service providers
The telematic means for the medical teleconsultant to remotely interact with the local multimedia medical guide and efficiently demonstrate health care procedures to local paramedics. Requests for help, broadcast over INMARSAT links will be routed to one or more specialists depending on type of help needed, language(s) required for efficient communication, proximity to the site of emergency etc. Thus MERMAID, relying on telepresence and combining locally stored multimedia reference material with remote consultation, proposes a viable solution to the problem of missing medical expertise for isolated populations.

It is significant that the MERMAID pilot users represent 1% of the world merchant marine, a size that guarantees the validity of its outcome and is a good start for disseminating the MERMAID results.

Results

The results of the project are the following:

Setting up of an integrated 24-hour multilingual, telematic, around-the-world, medical emergency service that serves as a generic model for telemedicine in the EU and as a pilot project within the Global Maritime Distress and Safety System (GMDSS).
Preventiion of the deterioration of medical emergencies into undenable situations through expert help provided via Telepresence. Highly efficient telemedical services based upon direct patient-doctor interaction, face-to-face consultation, visual inspection and better local medical skills.
MERMAID offers a viable alternative to locally missing medical expertise. It is a generic platform for developing remote applications (e.g., tele-inspection). By combining use of local resources with teleconsultation it improves productivity in remote environments.
MERMAID implements Council Directives 92/29 and 93/103, responds to the common G - 7 and EU policy on “Global Healthcare Applications” and provides a generic model for telemedicine contributing to the GMDSS.
